32 Participants Selected for CMS’ Accountable Health Communities Model

The Center for Medicare and Medicaid Innovation has announced 32 recipients of Accountable Health Communities (AHC) cooperative agreement awards in two of three tracks - Assistance and Alignment. Over a five-year period, the 32 participants will serve as local “hubs” to link clinical and community services. “CMS will implement and test the three-track AHC model to support local communities in addressing the health-related social needs of Medicare and Medicaid beneficiaries by bridging the gap between clinical and community service providers.” The social needs to be addressed include housing instability, food insecurity, utility needs, interpersonal violence and transportation.
