Each summer, applications are available from the CityMatCH office for teams willing and able to participate in the DaTA Institute.
- All City MatCH member health departments are eligible to apply apply to the 2008 - 2009 DaTA Institute (see criteria for alumni teams below).
- Teams must be convened by the CityMatCH member health department and led by the CityMatCH representative representative (**see criteria for alumni teams below).
- Each team must be composed of at least three Core members, who reflect the range of skills and responsibilities of the three data use areas (data, programs and policy).
- CORE TEAM members are those team members who are required to travel three times during the year to DaTA Institute activities to be held at selected sites. There is a limit of five Core team members.
- PROJECT TEAM members are other key individuals from the agency/community who do not travel but play an important role in the team project. There is no limit to the number of Project team members.
- Institute team members may be from any relevant organization including: the local health department, local/state government, community-based organizations, academia, managed care organizations, or hospitals, and they may include community leaders, service providers, consumers, or others instrumental to the team's and the project's success.
- Core team composition encompasses the three corners of the Data Use Triangle: data, program and policy:
- Data: Responsible for conducting statistical analyses and data manipulation
- Program: Expertise in planning, designing, implementing, monitoring and evaluating programs.
- Policy: Influences partnerships and decision making in the organization and the community.
- Team project is "real" and "doable." The project must be a priority for the team members' organizations and must be focused on a maternal and child health issue of importance to the community.
- Team demonstrates strong partnerships and connections within its community. Special consideration will be given to applications from teams that incorporate community members into their composition.
- Team demonstrates strong support and commitment of health department leadership to full participation and successful completion of the DaTA Institute.
- Team representative attended an information session:
- via phone July 21, 2008, 3:00 pm - 4:00 pm ET
- Submission of a Statement of Need is required from Alumni Applicants explaining why the health department would want to send an additional team (e.g. new leadership, new team members, new important project, large department, etc).
- Alumni teams will be selected based on the strength of the application and justification for wanting to participate again. The CityMatCH representative is the only team member allowed to participate a second time. All other team members must be new DaTA participants.
**If the City MatCH representative chooses not to participate as a Core team member, she/he must serve as a Project team member.
- Health Department Letter: A letter from the health official in your local health department or equivalent is required. This letter should articulate: 1) support for health department team leadership and participation in the Institute; and, 2) health department support for the team project component.

**Alumni health departments: Please submit a Statement of Need explaining reasons for sending a second DaTA team (e.g. new leadership, new team members, new important project, large department, etc).
- Team Member Profile: Contact information for each team member must be submitted. A followup
email will be sent to each identifi ed person requesting a signed supervisor consent form to
participate in the Institute.
